I, CD, hereby revoke my last will dated the ЕЕ day of ЕЕ, year and modify the same by codicil dated the ЕЕ day of ЕЕ, year and all other testamentary depositions made by me whatsoever hereinbefore and declare it to be my intention to die intestate.
